Home from Hospital

Being discharged from hospital can be a daunting experience, especially for an elderly or disabled person. Our Home from Hospital service aims to make this process as stress free and comfortable as possible.

What is Home from Hospital?

When individuals are discharged from hospital they can often encounter a lengthy wait for transport home, sometimes arriving late in the evening; and if family aren’t around it can be a challenging and tiring event.

Home from Hospital

With our Home from Hospital service, we will send a qualified carer to meet and greet your relative at their home when first arriving back. Our team will always ensure the person being discharged has arrived home safely giving relatives peace of mind, this may mean contacting the hospital or waiting for hospital transport to arrive.

Home from Hospital

Our carers will help the service user in every way possible, including arranging medication, unpacking bags, help with washing and dressing, or simply turning the heating on and ensuring there is food for breakfast.

The initial coming home from hospital can be a taxing time for those needing support, that’s why we endeavour to support and make the readjustment to post-hospital life as smooth as possible.
